Heart & Soul Hospice is a not-for-profit, faith-based, Medicare-certified hospice organization serving patients and families from its Farmington office. Its compassionate team helps individuals facing life-limiting illnesses experience greater comfort, dignity and quality of life while supporting the people who care for them.

Each patient receives an individualized plan of care tailored to their medical, emotional, social and spiritual needs. Services may include coordinated care with the patient’s physician, skilled nursing, hospice aide assistance, social services, medication and symptom management, spiritual support, volunteer companionship and other resources related to the terminal diagnosis.

Heart & Soul Hospice also recognizes that serious illness affects the entire family. Team members provide education, guidance and emotional support to caregivers throughout the hospice journey. Grief and bereavement resources remain available to help families process loss and adjust after the death of a loved one.

Through the We Honor Veterans program, Heart & Soul Hospice provides specialized recognition and support for veterans and their families. Its One More Time program creates meaningful experiences by helping patients fulfill life-enhancing wishes, reconnect with treasured memories, or enjoy one more special moment with the people they love. Vigil care may also be provided during the final hours of life so patients and families do not feel alone.
