DESCRIPTION:The St. Francois County Community Partnership\, in collaboration with Parkland Health Center and Unite Us\, invites community partners to attend an informational presentation focused on improving coordinated care and strengthening referral pathways across our region.\n\nProfessionals across education\, healthcare\, nonprofits\, social services\, and public safety frequently encounter barriers when attempting to connect individuals and families with needed resources.
